Appointments

Please call our office to make your appointment. When scheduling, please provide your insurance information and the purpose of your appointment so we may better prepare for your visit.

If you require us to complete DMV forms, please mention this to our receptionist as additional testing and examination with our optometrists may be required.

BEFORE YOUR APPOINTMENT:

To expedite your visit, you may either

  • Print and complete our Registration Form and bring it to your appointment OR

  • Fill out the Registration Form through our online portal:

    • Please provide your email address when you schedule your appointment. Instructions will be sent through your email and you will have 72 hours to activate your portal.

    • Please sign the Practice and Financial Policies form and email it back to our office.

ON THE DAY OF YOUR APPOINTMENT:

Please arrive for your appointment at your scheduled time and bring the following:

  • Current medications and eyeglasses/contact lenses including your prescription

  • ID and Insurance Cards

  • Referral or authorization if you have an HMO insurance

We accept most PPO and HMO insurances, as well as Medi-CAL and Medicare. However, we DO NOT accept the plans listed below. Patients with these insurance plans will be responsible for payment at the time of service.

SURGERY CENTERS

Simple procedures can be done in our office, however the majority of surgeries are performed in the following outpatient surgical centers. All sites provide cutting-edge technology and friendly, highly-skilled staff.

OAKLAND:

Walnut Creek:

Refractive surgeries (LASIK and PRK) are performed in UC Berkeley School of Optometry.
